Northeast Asia on edge ahead of possible North Korean missile test

(CNN) -- Countries in northeast Asia remained on edge Wednesday amid warnings from U.S. and South Korean officials that North Korea could carry out a missile test at any point.
Japan has deployed missile defense systems around Tokyo, some Chinese tour groups have canceled visits to North Korea, and U.S. radars and satellites are trained on an area of the Korean east coast where Kim Jong Un's regime is believed to have prepared mobile ballistic missiles for a possible test launch.
After weeks of belligerent threats and provocative gestures from Pyongyang, the situation on the Korean Peninsula is fragile.
Adm. Samuel J. Locklear, the top U.S. commander in the Pacific, said Tuesday that he couldn't recall a time of greater tension in the region since the end of the Korean War in the 1950s.

Before the two controversial long-range rocket launches that North Korea carried out last year, the reclusive regime gave ample warning to the world. But it is keeping everyone guessing about what it might do this time around.
Intelligence suggests that North Korea may be planning "multiple mission launches" in the coming days beyond two Musudan mobile missiles it has placed along its east coast, a senior Pentagon official told CNN on Wednesday.
The official did not have specifics on the numbers of other missiles and launchers spotted on U.S. imagery.
The official said the North Koreans are military "masters of deception" and may have planned all along to focus the world's attention on the Musudans while they plan multiple launches of other
missiles, which is a tactic they have used in the past.

Money is my security blanket – and sometimes a mirage

When I talk about money my voice gets higher. Occasionally I'll try and stand back from the conversation, see if we sound like students speaking through fascinators of weed smoke, destroyed by the notion that cash is, like, just a promise, passed from pocket to pocket. Occasionally I'll try to stand deep within it and read the spaces between the words. More often I just stop mid-sentence. It's hard.
Money is hard. It confuses and depresses and pulls us backwards on a Megabus to our old bedroom at our mum's house in a town we thought we'd left forever. We're encouraged to learn how to control our money, but so often it feels impossible. Today, more than ever, when we talk about money we're talking about anxiety and failure, and fear. I realised recently how money, for me, is security. So I save in a way my friends don't – if I teeter on the edge of overdraft I panic. For many people I know, saving is a privilege – they live hand to mouth (hence some of the problems with housing associations' shift to direct payments), while I, in part because numbers baffle me so profoundly, treat it as an electric blanket. A padlock. Something to help me sleep soundly, keep me safe. But none of us, with our bulimic wallets, is totally relaxed. More than half of us in our late 20s and early 30s (according to a new survey about consumer trends by Axa) blame our parents for not equipping us to deal with finances. I'd question this, loudly.
How could our parents have taught us how to manage our money, when the money of their youth is a different shape to the money of ours? The two are only distantly related – they don't speak to each other. Our money is the stepsister of their money's son's cousin. Theirs was big and shiny, money in the time of brand-new teenagers, of jobs for life. Theirs was accruing interest while they lived at home, moving out only when they married, into houses that turned to gold beneath their feet. And their parents, still insecure postwar, never spending more than they had, were literally rebuilding their cities, compared with us, encouraged to rebuild our kitchens every 18 months. Our parents, now grandparents, didn't realise they'd be expected to work through their 60s and 70s, both in banks and as babysitters.
Our money is often a mirage. Our money is a plastic card with a print of a koala on it. And it lives in a state of perpetual arousal – adverts seduce us constantly. I looked at the Made.com sale last year: now when I read the Guardian online, the sofas I never bought flicker by. I sent an email referring to season two of Louie, and now adverts for Netflix frame my inbox. To resist is an active state.
And unlike 50 years ago, there is no shame in being in debt. We start our adult life that way – students who started university last year are expected to owe £53,400 by the time they graduate. The dream, in fact, is to win a bigger debt, a mortgage, a prize that none of my friends can even conceive of. All their money is swallowed by rent, so they have no choice but to live in debt. The Axa survey says one in four Britons feels forced to make "major lifestyle changes" this year, such as forgoing holidays, due to financial pressures. But it's bigger than this: it's not lifestyle, it's LIFE – it's writing off the idea of having children, it's staying in an abusive relationship because there's nowhere else to go.
As our earnings drop (the average income of middle-class households has fallen by almost 9% since the start of the recession), how can we help ourselves? After campaigning by the founder of MoneySavingExpert.com, the new-draft national curriculum will see financial skills taught with maths. Will this change how we deal with debt? With benefit cuts? And how much power do we really have? I have a flash of driving in an AA instructor's car, knowing that at any point he'll put his foot down on the hidden clutch. Even if I'm driving, it's just an illusion of control.

Obama pushes immigration reform deadline as Congress signals progress

Barack Obama put pressure on Congress on Monday to introduce an immigration reform bill by April, saying the "time has come to fix once and for all" the broken system.
Speaking at the White House at a naturalisation ceremony for 28 people, Obama indicated that he wanted to stick to the timetable he outlined in his inaugural address in January.
Members of Congress directly involved in discussions had been reporting better-than-expected progress over the last few weeks but by the end of last week admitted they had been unable to resolve outstanding differences before breaking for the Easter holiday.
Senators expressed hope that they could have a bill ready within two weeks of their return. Hopes of a deal have been raised by the support coming from Republican senators such as Marco Rubio, Rand Paul, John McCain and Lindsey Graham.
Speaking on Monday, Obama said: "We've known for years that our immigration system is broken, that we're not doing enough to harness the talent and ingenuity of all those who want to work hard and find a place in America. And after avoiding the problem for years, the time has come to fix it once and all. The time has come for comprehensive, sensible immigration reform."
He added: "We are making progress, but we've got to finish the job, because this issue is not new. Everyone pretty much knows what's broken, everyone knows how to fix it … So I expect a bill to be put forward. I expect the debate to begin next month. I want to sign that bill into law as soon as possible."
Immigration reform is among the key issues Obama wants to see addressed in his second term. Given the importance of the Latino vote to Obama's victory in November, the Republicans are more inclined to move on immigration reform than they are on other issues, such as gun laws.
Bipartisan groups in the Senate and the House are working on different versions of immigration reform. The Senate version broadly accepts the idea that reform should contain a pathway to citizenship for the estimated 11 million undocumented workers in the US, but there remains resistance to this among House Republicans, reluctant to reward undocumented workers that entered the US illegally.
Obama, making the case for immigration reform, said: "The point is that unless you are one of the first Americans, unless you are a Native American, you came from someplace else. That's why we've always defined ourselves as a nation of immigrants. And we've always been better off for it."
He reiterated that he regarded reform as tackling the legal immigration system so that families do not have wait years to join someone already in the country and a pathway to citizenship for undocumented workers that includes a background check, learning English, paying taxes, including a penalty, and going to the back of the queue behind everyone trying to get to the US legally.

Acne Treatments and drugs

Acne treatments work by reducing oil production, speeding up skin cell turnover, fighting bacterial infection, reducing the inflammation or doing all four. With most prescription acne treatments, you may not see results for four to eight weeks, and your skin may get worse before it gets better.
Your doctor or dermatologist may recommend a prescription medication you apply to your skin (topical medication) or take by mouth (oral medication). Oral prescription medications for acne should not be used during pregnancy, especially during the first trimester.
Types of acne treatments include:
  • Over-the-counter topical treatments. Acne lotions may dry up the oil, kill bacteria and promote sloughing of dead skin cells. Over-the-counter (OTC) lotions are generally mild and contain benzoyl peroxide, sulfur, resorcinol, salicylic acid or sulfur as their active ingredient. These products can be helpful for very mild acne. OTC acne medications may cause initial side effects — such as skin irritation, dryness and flaking — that often improve after the first month of therapy.
  • Topical treatments available by prescription. If your acne doesn't respond to OTC treatments, consider seeing a doctor or dermatologist to get a stronger prescription lotion. Tretinoin (Avita, Retin-A, others), adapalene (Differin) and tazarotene (Tazorac, Avage) are examples of topical prescription products derived from vitamin A. They work by promoting cell turnover and preventing plugging of the hair follicles. A number of topical antibiotics also are available. They work by killing excess skin bacteria.
Often, a combination of such products is required to achieve optimal results. A number of benzoyl peroxide and antibiotic combination medications are available, including different dose combinations of benzoyl peroxide and clindamycin (Benzaclin, Duac, Acanya) and benzoyl peroxide and erythromycin (Benzamycin). Dapsone gel (Aczone) is a newer acne treatment that's particularly effective in treating inflammatory acne. Prescription topical treatments for acne may cause skin side effects, such as stinging, burning, redness or peeling. Your doctor may recommend steps to minimize these side effects, including using a gradually increased dose, washing off the medication after a short application or switching to another medication.
  • Antibiotics. For moderate to severe acne, you may need a short course of prescription oral antibiotics to reduce bacteria and fight inflammation. Since oral antibiotics were first used to treat acne, antibiotic resistance has increased significantly in people with acne. For this reason, your doctor likely will recommend tapering off these medications as soon as your symptoms begin to improve, or as soon as it becomes clear the drugs aren't helping — usually, within three to four months. In most cases, you'll use topical medications and oral antibiotics together. Studies have found that using topical benzoyl peroxide along with oral antibiotics may reduce the risk of developing antibiotic resistance. Antibiotics may cause side effects, such as an upset stomach, dizziness or skin discoloration. These drugs also increase your skin's sun sensitivity and may reduce the effectiveness of oral contraceptives.
  • Isotretinoin. For deep cysts, antibiotics may not be enough. Isotretinoin (Amnesteem, Claravis, Sotret) is a powerful medication available for scarring cystic acne or acne that doesn't respond to other treatments. This medicine is reserved for the most severe forms of acne. It's very effective, but people who take it need close monitoring by a dermatologist because of the possibility of severe side effects. Isotretinoin is associated with severe birth defects, so it can't be safely taken by pregnant women or women who may become pregnant during the course of treatment or within several weeks of concluding treatment. In fact, the drug carries such serious potential side effects that women of reproductive age must participate in a Food and Drug Administration-approved monitoring program to receive a prescription for the drug.
Isotretinoin commonly causes side effects — such as dry eyes, mouth, lips, nose and skin, as well as itching, nosebleeds, muscle aches, sun sensitivity and poor night vision. The drug may also increase the levels of triglycerides and cholesterol in the blood and may increase liver enzyme levels.
In addition, isotretinoin may be associated with an increased risk of depression and suicide. Although this causal relationship has not been proved, doctors remain on alert for these signs in people who are taking isotretinoin. If you feel unusually sad or unable to cope while taking this drug, tell your doctor immediately.
  • Oral contraceptives. Oral contraceptives, including a combination of norgestimate and ethinyl estradiol (Ortho Tri-Cyclen, Previfem, others), can improve acne in women. However, oral contraceptives may cause other side effects — such as headaches, breast tenderness, nausea and depression — that you'll want to discuss with your doctor. The most serious potential complication is a slightly increased risk of heart disease, high blood pressure and blood clots.
  • Laser and light therapy. Laser- and light-based therapies reach the deeper layers of skin without harming the skin's surface. Laser treatment is thought to damage the oil (sebaceous) glands, causing them to produce less oil. Light therapy targets the bacteria that cause acne inflammation. These therapies can also improve skin texture and lessen the appearance of scars. More research is needed to understand the most effective use of light and laser therapies in acne treatment, and experts currently recommend these approaches as stand-alone therapy only in people who can't tolerate approved acne medications. These therapies may be uncomfortable and may cause temporary skin problems that mimic a severe sunburn.
  • Cosmetic procedures. Chemical peels and microdermabrasion may be helpful in controlling acne. These cosmetic procedures — which have traditionally been used to lessen the appearance of fine lines, sun damage and minor facial scars — are most effective when used in combination with other acne treatments. They may cause temporary, severe redness, scaling and blistering, and long-term discoloration of the skin.
Acne scar treatment
Doctors may be able to use certain procedures to diminish scars left by acne. These include fillers, dermabrasion, intense light therapy and laser resurfacing.

Soft tissue fillers. Collagen or fat can be injected under the skin and into scars to fill out or stretch the skin, making the scars less noticeable. Results from this acne scar treatment are temporary, so you need to repeat the injections periodically.
Chemical peels. High-potency acid is applied to your skin to remove the top layer and minimize deeper scars. Some stronger peels reach even deeper into the skin.
Dermabrasion. Usually reserved for more severe scarring, dermabrasion involves removing the top layer of skin with a rapidly rotating wire brush. Surface scars may be completely removed, and deeper acne scars may appear less noticeable. Dermabrasion may cause pigmentation changes for people with darker skin.
Microdermabrasion. This newer acne scar treatment involves a hand-held device that blows crystals onto skin. These crystals gently abrade or "polish" the skin's surface. Then, a vacuum tube removes the crystals and skin cells. Because just the surface cells are removed, the skin isn't damaged. However, results are subtle and scars may still be noticeable, even after several sessions.
Laser, light source and radiofrequency treatments. In laser resurfacing, a laser beam destroys the outer layer of skin (epidermis) and heats the underlying skin (dermis). As the wound heals, new skin forms. Less intense lasers (nonablative lasers), pulsed light sources and radiofrequency devices don't injure the epidermis. These treatments heat the dermis and cause new skin formation. After several treatments, acne scars may appear less noticeable. This means shorter recovery times, but treatment typically needs to be repeated more often and results are subtle.
Skin surgery. A minor procedure (punch excision) cuts out individual acne scars. Stitches or a skin graft repairs the hole left at the scar site

Structured settlements

For over a quarter-century, MetLife has provided financial security through structured settlement annuities for the claimants of personal injury settlements. Our structured settlement annuities are offered exclusively through structured settlements specialty brokers who look to MetLife for our expertise and ability to structure exactly the right settlement for the situation.

The Benefits of MetLife Structured Settlements include:

Tax-free income. A structured settlement is a one-time opportunity to settle a physical injury claim (including wrongful death) and/or workers' compensation claims, with a future stream of tax-free benefit payments.1 Section 104(a)(2) of the Internal Revenue Code ("Code") clarifies that the full amount of the structured settlement payments related to personal injury claims is tax-free. Code Section 104(a)(1) provides similar tax relief for workers' compensation awards. By contrast, the investment earnings on a lump sum payment are usually fully taxable.2
Customized payment streams. A MetLife structured settlement can be tailored to meet a claimant's specific needs. Payments can be scheduled up to 40 years in the future and MetLife can pay a claimant over his/her entire lifetime. Payments can be made in equal amounts or can vary over time, and can begin immediately or be deferred. Payments can also be designed with escalators and benefit increases to address future inflation concerns, and may be scheduled to include future lump sum payouts for things such as necessary medical care or equipment, a down payment on a house, or educational needs for children.3
Payments for as long as the claimant lives. In times of increasing longevity, when many people are concerned about outliving their savings, the availability of lifetime payments can be of critical importance to claimants.  Lifelong income can provide a claimant with increased financial security.
Payments to the beneficiary. If a claimant accepts an all-cash settlement, there is no assurance that funds will remain after death for a named beneficiary. However, guaranteed structured settlement payments can be received by the named beneficiary.

What Causes Peritoneal Mesothelioma?

What Causes Peritoneal Mesothelioma

Peritoneal mesothelioma is known only to be caused by exposure to asbestos, a microscopic natural fiber that was used heavily in industry. Asbestos is said to reach the abdominal wall by one of two methods. The first is through ingested asbestos fibers which are processed through digestion and become lodged in the peritoneum. The other method is through the lungs and lymph nodes, by inhaled asbestos fibers. Malignant mesothelioma of the pleura is also known to metastasize directly into the abdominal cavity if its spread is not slowed.
Many of those who have worked with asbestos materials have developed peritoneal mesothelioma as a result of exposure to friable asbestos fibers. Asbestos was used in myriad industrial materials and construction products. Exposure to asbestos was common in aluminum plants, steel mills, power plants, and naval shipyards.
Peritoneal mesothelioma is a relatively rare disease, with only 200-500 diagnoses each year in the United States being documented. Risk however, is sustained in those who have been exposed to asbestos. Even those exposed many years ago are potentially at risk for the development of malignant peritoneal mesothelioma. Latency between exposure to asbestos and the development of peritoneal mesothelioma symptoms can be anywhere from 20-50 years. Misdiagnosis is also common because peritoneal mesothelioma symptoms closely mimic minor stomach/abdominal discomfort and hernia. Nevertheless, those with clear asbestos exposure history should be aware of the disease and speak with their doctor or cancer specialist if they believe they may be experiencing symptoms related to mesothelioma.

How is Peritoneal Mesothelioma Treated

Prognosis of mesothelioma patients is poor, and peritoneal mesothelioma patients are no exception. Most peritoneal treatment regimens for patients include palliative methods such as mesothelioma chemotherapy and radiation therapy. However, if the disease is diagnosed in its earlier stages, patients such as those diagnosed with stage 1 mesothelioma, may be eligible for surgical resection of the disease, which could potentially add years to the initial prognosis.
Surgery is possible for patients diagnosed with earlier-stage disease. Some surgical options include peritonectomy and palliative procedures. Certain physicians, including Dr. David Sugarbaker of the International Mesothelioma Program at Brigham and Women’s Hospital in Boston, MA, have become extremely well versed and experienced in the surgical treatment of malignant mesothelioma. In many cases, surgical treatment for mesothelioma will be coupled with chemotherapy or radiation therapy regimens.
Chemotherapy is the most common treatment for mesothelioma disease. Currently, there is only a single chemotherapy regimen (Alimta® (pemetrexed) in conjunction with Cisplatin) approved by the Food and Drug Administration as a prescribed therapy for the treatment of malignant pleural mesothelioma. There are however, dozens of clinical trials being conducted for mesothelioma patients experimenting with new drugs, including Gemcitabine, Navelbine, Onconase, and others for the treatment of malignant peritoneal mesothelioma.

Prevention of mesothelioma

Mesothelioma Prevention

For people who have worked with asbestos for many years, the discussion of mesothelioma prevention seems to be a moot point. Many who have been exposed to asbestos believe it's already too late to talk about ways to avoid developing mesothelioma. They think the affect of asbestos on the lungs has already manifested itself and there's no escaping the threat of developing the disease. However, past asbestos exposure is not the only factor people should worry about because the possibility of being exposed today is just one mistake away.

Asbestos was used in literally thousands of products throughout much of the twentieth century. Many of those items, particularly building products, are still in homes, offices, and factories across the country. Workers in specific industries can still encounter asbestos as well, including those who work in shipyards, power plants, chemical plants, or in the railroad and automotive industry

Prevention at Work

In the past, the majority of workers had no idea that asbestos exposure could lead to the contraction of serious diseases. Today, however, most individuals are aware of the dangers, especially if they work in an industry that traditionally made use of the toxic substance. Regardless of the knowledge that is now available, not everyone takes the proper precautions that are needed to prevent exposure.

Preventing asbestos exposure and the risk of contracting mesothelioma at work can be accomplished through a few simple precautions. First of all, protective gear should be worn any time the presence of asbestos is suspected. Secondly, any clothes worn while working with asbestos should be left at the site. Wearing asbestos-covered clothes outside the work area could subject others to unwanted exposure. When dealing with asbestos, proper abatement methods should also be followed to ensure complete safety

Prevention in the Home

Most people don't encounter asbestos simply by walking around their home even if there is asbestos inside. Asbestos exposure usually occurs during home renovation projects because do-it-yourself projects can catch non-professionals off guard.

Some of the in-home items that may contain asbestos include:

Attic insulation
Roof shingles and tar
Drywall and drywall glue
Floor tiles
Popcorn ceilings
Joint compounds

Wrapping on pipes and electrical wires
Asbestos that is already damaged or may be crumbling because of age is referred to as "friable" asbestos. This kind of asbestos is especially dangerous because the toxic fibers that make up asbestos can easily circulate through the air become inhaled. Anyone performing any type of serious renovation work on a home built prior to 1980 should always wear protective gear

Mesothelioma Prevention After Exposure

Individuals who have already been exposed to asbestos, but have not developed an asbestos-related disease should be diligent about monitoring their health on a regular basis.

This can be done in a few different ways:

Mesothelioma blood test - A simple blood test that can detect mesothelioma before its symptoms appear was approved by the FDA. Dubbed Mesomark, this test can be administered in a doctor's office. Find out if your doctor has access to the test and be sure to be tested annually.
Imaging and other tests - Asbestos-exposed individuals can also be monitored via a diagnostic imaging test such as an x-ray, or by means of a pulmonary function test, which measures how well the lungs are working. These should also be done annually
Also, anyone who has been exposed to asbestos and has not yet developed an asbestos-related disease should not smoke. Smoking greatly increases the possibility of developing such a disease. In addition, those who have asbestosis and continue to smoke run a much higher risk of developing mesothelioma.
Joint compound
Roofing tar
Brake pads
Fireproofing
Pipes
Insulation
Countertops
Floor tiles
Textured wall and ceiling coatings
If these items are intact, they usually present little to no health hazards. However, if they are damaged or "friable," they should be removed from the house immediately. Removal should be done by a licensed abatement company, as this is the best way to protect you and your family from asbestos exposure.

On-the-Job Prevention

Workers in particular industries may still be at risk for exposure to asbestos. Those in the construction industry, for example, may encounter old buildings that contain asbestos. It is necessary to take great care in protecting yourself if the presence of asbestos is suspected, which includes wearing proper gear to avoid inhalation.

Monitoring Your Health

If you have already been exposed to asbestos, regularly monitoring your health is essential in order to catch the early warning signs of disease. Remember, treatment options are greater the earlier the disease is diagnosed. It is important to have regular chest x-rays and pulmonary function tests, even if you are experiencing no symptoms of disease.
In addition, if you have been exposed to asbestos in the past, you may be able to reduce your chances of developing mesothelioma by not smoking. Studies show that smoking damages the lungs and can affect the risk of developing mesothelioma. Anyone who does develop an asbestos-related disease should stop smoking immediately
